Consumer Portal

West Michigan Community Mental Health (WMCMH) offers a free consumer portal that provides convenient, 24-hour access to personal health information from anywhere with an internet connection. Using a secure username and password, consumers may view their WMCMH records such as medication list, contact information and service history. The portal also allows consumers to send and receive non-emergency messages to their care manager.

To create an account, WMCMH will give you a case number and a Personal Identification Number (PIN). To request this information, please call 1-800-992-2061.
