Support for Older Adults with Behavioral Health Concerns Across Mason, Lake, and Oceana Counties
West Michigan Community Mental Health (WMCMH) Older Adult Services supports adults age 60 and older by helping connect them to mental health services, healthcare resources, community supports, and programs that promote independence and overall wellness.
Our Older Adult Care Manager/Supports Coordinator works alongside individuals, families, healthcare providers, and community organizations to help older adults navigate services and access the support they need.
Who Can Access Older Adult Services?
Older Adult Services are available to adults age 60 and older within the WMCMH service area, including Mason, Lake, and Oceana counties.
Services may benefit older adults experiencing:
- Mental health concerns
- Co-occurring substance use disorders
- Intellectual or developmental disabilities
Services May Include
Services are person-centered and individualized based on each person’s needs and goals. Support may include:
- Care coordination and service planning
- Connecting individuals to community resources
- Assistance navigating healthcare and Medicaid systems
- Coordination with healthcare providers and agencies
- Monitoring service effectiveness and satisfaction
- Advocacy and ongoing support
- Connections to transportation resources
- Referrals to senior centers, Commission on Aging programs, and other local supports
